Havent tried this one, though I suppose it doesn't have to be a form
provided you quote your client number and callsign in case they have
others with a similar name. An email may suffice. Your ham licence
requires you to nominate the base address where you operate from.

If you receive the monthly "Amateur Radio" magazine, then you are
(unless you specially opted not to get it). You can contact them by
email if you are not sure.
Again a special form would not be necessary. These are generally only
used where a business is dealing with many thousands of members of the
public to ensure they include account numbers etc to avoid ambiguity.
For the WIA I guess an email would suffice.
